This form must be accomplished for the processing of your Letter of Authorization for medical consultations.
REMINDERS:
- Please be reminded that processed Letters of Authorization must be used within three (3) days from approval.
- Please allow us at least 24 hours to process your request once submitted.
- Requests submitted beyond operating hours (6am – 6pm) will be processed on the next business day.
